Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is one of the best, or even the only, option for the cure of leukemia, especially for patients with high risk factors.(1) However, it is limited by the shortage of suitable donors, because only 25%-30% patients can find a human leukocyte antigen (HLA)-identical sibling donor. Although much progress has been made in finding a suitable unrelated donor or unrelated cord blood (UCB) due to the expansion of the worldwide unrelated donor program, the shortage of suitable donor remains an important issue for several reasons. The probability of finding a matched unrelated donor ranges from less than 10% in ethnic minorities to 60%-70% in Caucasians.(2) The complicated process in searching for an unrelated donor has an average of about 4 months from initiation of a search to the donation of stem cells. During this time some patients might relapse, even die while waiting for the transplantation.(3) Allo-HSCT using a matched unrelated donor is still associated with a high transplant-related mortality (TRM) and high long-term morbidity.(4,5) UCB transplantation offers the advantages of easy procurement, immediate availability, the absence of risk to donors, and the reduced risk of graft-versus-host disease (GVHD).(6) However, engraftment remains a significant problem, especially for adult patients.
