摘要: [目的]分析髋关节置换骨水泥型股骨假体周围水泥鞘的质最和厚度.[方法]回顾北京大学第三医院骨科自2004年2月-2007年12月采用相同入路、操作技术和假体完成的262例(279髋)初次髋关节置换病例,分析术后出院前X线片,对股骨侧骨水泥鞘质量和厚度进行分析.[结果](1)按Barrack标准,骨水泥质量A级87.1%,B级11.8%,C级1.1%,无D级骨水泥;(2)按Gruen分区,骨水泥鞘厚度<2 mm区域多见于正位片上1、2、5区和侧位片上8、9、12区;透亮带多位于假体中远端的2~5区和10~12区;气泡影出现在假体远端的3~5区和10~12区.(3)股骨假体和股骨解剖轴线在正位片上的夹角,在骨水泥鞘厚度>2 mm组中为1.46°±0.75°(-1.60°~3.41°);在骨水泥鞘厚度<2 mm区域组中为3.90°±1.52°(-2.99°~7.69°),两组间差异有统计学意义(P<0.05).[结论]正确使用第3代骨水泥技术,可获得可靠的骨水泥固定质量.股骨假体应参考股骨解剖轴线中立位置入,以期获得均匀一致,足够厚度的骨水泥鞘. [Objective]To evaluate the quality and thickness of the cement mantle of femoral components in primary hip arthroplagty.[Method]Radiographic results of 262 patients(279 hips)with primary hip arthroplagty from February 2004 to December 2007 were postoperatively reviewed.It was completed with the same approach,cementing technique and prosthesis.[Resuit]According to Barrack's classification criteria for cement mantle quality,87.1% artificial hips were classified as grade A,11.8%were grade B and 1.1% were grade C.There was a high risk of thin cement mantle (<2mm)in Gruen zones 1,2 and 5 in the anterior-posterior plane and in Gruen zones 8,9 and 12 in the lateral plane.Typical areas of radiolucency were identified in Gruen zones 2~5 and 10~12 in the middle-distal of the stem.Voids appeared in Gruen zones 3~5 and 10~12 in the distal of the stem.On the anterior-posterior view,stem alignment was 1.46°±0.75°(-1.60.-3.41°)on the group of cement mantle thickness above 2mm,and 3.90°±1.52°(-2.99°-7.69°)in the group of below 2 mm.There was significant difference between the two groups(P<0.05).[Conclusion]The results of using the third generation cementing techniques produced reproducible and reliable quality of the cement mantle on femoral side in primary hip arthroplasty.A neutral position of the shaft of the femoral component is optimal for even more cement mantle.
