摘要: 全机器人不停跳冠状动脉旁路移植术创伤小,术后恢复快,是未来微创冠状动脉外科的趋势,对麻醉管理提出更高的要求,主要包括加压CO2气胸、单肺通气的应用、经食道超声心动图的监测、术中血流动力学的维持、体外循环及中转开胸手术的准备、室颤的处理等,要求麻醉医师对患者病理生理的调控及围术期的管理达到更精细化。 [Summary] Robotic totally endoscopic coronary artery bypass on beating heart (BH-TECAB), a minimally invasive surgical approach with rapid postoperative recovery , is becoming the future trend of minimally invasive coronary surgery , which demands a higher standard for anesthetic management . It comprises pressured pneumothorax , unilateral lung ventilation , transesophageal echocardiography monitoring , hemodynamics maintaining , preparation for cardiopulmonary bypass and conversion to sternotomy , and ventricular fibrillation treatment , which require anesthesiologists to achieve more refined perioperative management and pathophysiological regulation .
